The Neuro Nation Network

Neuro Nation was created by Dr. Chloe Giordano in 2022 to assist individuals in regaining independence, promote a sense of community, provide knowledge to patients and caregivers, and promote hope. 

Dr. Giordano saw a gap in neurologic physical therapy treatment where patients became a “revolving door.”  During patient's physical therapy program they would improve, finish physical therapy to figure it out on their own, then experience a decline in function (which may include a hospital stay) and require physical therapy all over again. Dr. Giordano wanted to stop this cycle by creating a long term follow up plan to ensure patients don’t get stuck in the revolving door trap!

Our goal is to facilitate long term wellness after your neurologic physical therapy treatment to prevent avoidable hospitalizations that can cause a decline in abilities and financial strain. We make it easy for you by coming to you in the comfort of your home, nursing home, or assisted living facility.

Meet our Team!

Dr. Chloe Giordano, PT

Dr. Chloe Giordano was born in New York and played Division 1 softball at BGSU in Ohio. She received her Doctor of Physical Therapy (DPT) from Elon University in North Carolina and went on to train at Brooks Rehabilitation in Jacksonville, FL. Brooks Rehab is an inpatient rehab hospital and CARF-accredited  in stroke, brain injury, spinal cord injury, pain, and general medical rehabilitation. She has over 12 years in exercise training and over 4 years of specialized physical therapy knowledge. 

In her free time Dr. Giordano loves to spend time with her family and dogs, visit local coffee shops, hike, garden, try new foods, cook, and enjoy the outdoors.

Dr. Alys Giordano, PT, NCS

Dr. Alys Giordano was born in Ohio and studied exercise science at Slippery Rock University. She received her Doctor of Physical Therapy (DPT) from Elon University and is an adjunct professor at Elon's DPT program teaching in the  Neurologic and Geriatric courses. 

Dr. Alys Giordano is board certified in neurologic physical therapy receiving her neurologic specialist certification (NCS). She is currently working on her PhD in healthcare administration and education.
